Using the Referral Partners Tool in CareFunnels

The Referral Partners tool helps you track which businesses are sending you referrals, manage relationships with individuals at those businesses, and measure whether those partnerships are producing results. Access it by clicking Referral Partners in the left navigation menu.


Adding a Referral Partner

A referral partner represents an organization — such as a hospital, assisted living facility, or physician's office — that sends clients your way.

  1. Go to Referral Partners in the left navigation to see your list of existing partner organizations.

  2. Click Add Referral Partner in the top-right corner.

  3. Enter the organization name and click Save.

  4. Fill in additional details: organization type (hospital, assisted living, etc.), account status (prospecting, active, inactive), referral potential, phone number, address, and any notes.

  5. Click Save. Refresh the page and the new partner will appear in your list.

Tip: You can add notes and tasks directly from the referral partner record. Use notes to log meeting outcomes and tasks to schedule follow-ups.


Adding Contacts to a Referral Partner

Contacts are the individual people you work with at a partner organization — care coordinators, discharge planners, social workers, etc.

  1. Go to the Contacts page and find the person you want to associate (or create a new contact).

  2. Click their name to open their record.

  3. In the top-right of the record, click the Associations icon (three overlapping circles).

  4. Under Referral Partners, search for an existing partner or create a new one, then select the correct organization.

  5. The contact is now linked. Their notes and activity will be visible from the referral partner's record.

Tip: Once linked, notes logged on a contact will appear in the partner's notes feed when you filter to show associated contacts — keeping your full relationship history in one place.


Associating Clients with a Referral Partner

Tracking which clients came from which partner helps you measure the value of each relationship over time.

  1. Navigate to Client Intake (or wherever the client opportunity lives in your pipeline).

  2. Click the client's opportunity to open it.

  3. In the Associated Objects section, find the Referral Partners field and select the appropriate partner organization.

  4. Click Save.

To review results, open the referral partner's record and go to the Associations tab. You'll see all opportunities linked to that organization — a quick way to evaluate whether a partnership is worth continued investment.


Using the Referral Partners Pipeline

CareFunnels includes a dedicated Referral Partners pipeline for managing the relationship-building process with contacts at partner organizations before they become active referral sources. Move contacts through stages as the relationship develops: Identified → First Contact Made → Meeting Scheduled → First Referral Received → Active Partner.

Once you receive a first referral, create the corresponding referral partner organization and begin tracking clients from that source. The pipeline ensures no relationship falls through the cracks when you're managing multiple prospects at once.


Viewing Notes and Activity

From any referral partner record, you can view notes two ways:

Partner-level notes: The default view shows notes logged directly on the referral partner record — meeting summaries, general observations, etc.

Notes from associated contacts: Use the filter on the notes tab to include activity from linked contacts. This surfaces all your touchpoints — calls, visits, emails — in one feed alongside your organization-level notes.
